Child Labor Permit Information

Child Labor Permits are issued BY APPOINTMENT ONLY

Appointments scheduled between the hours of 8:15 AM – 4:00 PM Monday through Friday (excluding holidays). Call 715-386-4607 or 715-386-4608 to schedule an appointment.

The minor MUST be present and bring the following documents:

  • Proof of Age: WI state identification, birth certificate or baptismal record
  • Social Security Card: minor must present the physical card as requested by Wisconsin State Law
  • Letter from the Employer: signed letter of hire on the company's letterhead, including job duties, hours of work, and the time of day the minor will be working
  • Written Consent from Parent/Guardian: signed letter giving consent to the minor to work

Effective June 23, 2017--16- and 17-year-old minors do not need to obtain a Child Labor Permit.
