This survey was created to get our community's feedback on factors that impact health in Pierce or St. Croix County. The results will be used to develop our Community Health Improvement Plan for 2026 through 2030. By participating, you ensure that our efforts address the real needs and concerns of the residents in our counties.
We need your help to get a true sense of what health related issues our community is facing, and that means hearing from everyone. Whether you're a long-time resident or new to the area, your experiences and insights are important.
How to Participate

You can complete the survey online or paper copies will be available at St. Croix County Public Health and local libraries. This survey is meant for residents of Pierce and St. Croix County ages 18 years old and up. Your responses are confidential.
