St. Croix and Pierce County Public Health Departments are conducting a Community Health Survey to gather our community's insight on factors that impact health in Pierce or St. Croix County. The results will be used to develop our Community Health Improvement Plan for 2026 through 2030. By participating, you ensure that our efforts address the real needs and concerns of the residents in our counties.
We need your help to get a true sense of what health related issues our community is facing, and that means hearing from everyone. Whether you're a long-time resident or new to the area, your experiences and insights are important.
How to Participate
Don’t wait – share your thoughts today! It is easy and takes just a few minutes. This survey is meant for all residents of Pierce and St. Croix County ages 18 years old and up. Your responses are confidential.
