Special Event Food Service Application

  1. This application is for Saint Croix County special event food service only. Examples of special events include fairs, carnivals, festivals, special promotions events, and farmer’s markets. All special event food service operators shall complete this application. If you are an operator that operates independent of such special events, this application is not the correct form and call 715-246-8361 for assistance.

    All special event food service operators must complete this form and submit 7 days in advance of the first event listed.

  2. Section 1-Stand Information

  3. What Saint Croix County event(s) do you plan to operate ?

    Note: License year is July 1 to June 30 of each year.

  4. Please include the name of event, location, date(s) and time you will be set up.

  5. Section 2: Determing if the stand is exempt from licensing

  6. The state code does exempt some food stands from the requirement of purchasing a license. There are two categories of exemptions. One category are for those stands that meet the listed criteria and meet the definition of a restaurant. The second category are for those that meet the listed criteria and meet the definition of a retail food establishment.

    Meals such as pasties, Corn dogs. hot dogs, brats, burgers, pizza, cooked fruits/veggies

    Criteria for Exemptions:

    -Foods stands at youth sporting events that are operated by the youth organization.

    -Civic organizations that operate less than 4 days per year.

  8. Retail Food Establishment Stand Type:

    Non-meal type items such as bakery, confectionery, soft drinks, ice cream, milk, coffee, tea, pastry items, sliced fruit/veggies, nuts

    Criteria for Exemptions:

    Religious, charitable, or non-profit organizations that operate less than 12 days per year

    Stands that are exempt from licensing will be provided a food safety checklist that should be used in setting up your food stand to ensure safe foods are provided to the public.

  11. Section 3: Licensing

  12. If no, you are elibigle to be licensed by St. Croix County. Using the License Table above, determine whether your stand is a "restaurant" or "retail food" establishment. Then determine license category and approriate fee.

  13. Section 4- Signature

  14. Your signature below acknowledges that you have reviewed the Special Event Requirement Checklist. Your signature certifies the above-described establishment will be operated and maintained in accordance with applicable regulations.

  15. Electronic Signature Agreement

    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
